Notice is hereby given that a Deed Poll dated 24 October 2024 and enrolled in the Senior Courts of England and Wales on 24 March 2025, I, Monika Hornak, 18 Post Drive, Stowupland, Suffolk IP14 4FL, Married/Civil Partnership and a British Citizen, under section 1(1) of the British Nationality Act 1981 abandoned the name of Monika Hornakova and assumed the name of Monika Hornak.
