Notice is hereby given that a Deed Poll dated 08/04/2024 and enrolled in the Senior Courts of England and Wales on 24/10/2025, I, Sam Michael Fordlay, 90 Avonvale Road, Redfield, Bristol, BS5 9RU Married/Civil Partnership and a British Citizen under section 1(1) of the British Nationality Act 1981 abandoned the name of Samuel Michael John Ford and assumed the name of Sam Michael Fordlay.
